UPPER DECK PROMOTIONS

Upper Deck is offering hobby collectors the following two bonus card programs beginning on August 15, 2023, through August 14, 2024 (“Bonus Card Period”) to receive a Cole Caufield autograph rookie card (collectively the “Bonus Cards”):

2021-22 Credentials (“Credentials Bonus Card”) 2021-22 Stature (“Stature Bonus Card”)

Collectors who redeem select cards, as noted below, from one or both of the 2021-22 Credentials and/or 2021-2022 Stature products will receive one (1) randomly selected Credentials Bonus Card and/or Stature Bonus Card.

Please complete and enclose the Bonus Card Form below along with the Submission. By submitting the Bonus Card Form and/or Submission above, you acknowledge and agree to Upper Deck’s Privacy Policy found at UpperDeck.com and these redemption terms. Upper Deck is not responsible for any lost, stolen, or damaged Submission or Bonus Cards while in transit to or from Upper Deck. All Submissions must be received by Upper Deck within the Bonus Card Period.

To save and print the Bonus Card Form: (1) click the image (2) once you see the border around it, right-click (3) save image (4) print image Once your Submission has been received and reviewed by Upper Deck, one (1) randomly selected Bonus Card will be mailed to you. Bonus Cards will be shipped within approximately four to six weeks from Upper Deck’s review of the Submission.

Following the completion of the Bonus Card Period (on or after August 14, 2024), your Submission (including the 2021-22 Credentials redemption set and/or one of the 2021-22 Stature redemption cards) will be returned to the name and address noted on the Bonus Card Form. If there is an address change during the Bonus Card Period, please send an email, including the new mailing address to Upper Deck at QA@upperdeck.com. This bonus card program is limited to individuals who are legal residents of the fifty (50) United States and the provinces and territories of Canada (excluding Quebec) who have reached the age of majority in their jurisdiction of residence and who are at least eighteen (18) years old at the time of submission.
